Location: Ditchling, Mid Sussex
Salary: £28K-£30K Depending on experience
Hours: 40 hours + On call (rota basis) Hours 8.30am - 5pm

Your role:
As a Care Coordinator you’ll be an integral and fundamental part of our service – managing schedules, supporting Care Assistants and ensuring every client receives outstanding care. This is a fast paced role for someone organised, personable and committed to excellence.
What we are looking for:
Experience in care coordination or similar (essential)
Strong communication and organisational skills
Ability to build strong relationships with clients, families, and colleagues.
Empathy and resilience under pressure
Problem-solving mindset and attention to detail
Tech-savvy with scheduling systems
A full UK driving licence and own vehicle
Self-motivated and flexible, with a willingness to participate in an on-call system for out of office hours
